Netherite Helmet is Minecraft's top-tier head armor, upgraded from a Diamond Helmet for +3 armor toughness, knockback resistance, and lava-proof item survival.
Description
Netherite Helmet keeps every enchantment from the Diamond Helmet it was forged from, so Protection IV, Respiration, and Aqua Affinity carry over for free. As a netherite item, Netherite Helmet floats in lava and does not burn when dropped on the ground, making it far harder to lose during Nether mining trips. Pair Netherite Helmet with a full netherite set for the maximum 80% damage reduction at 20 armor points, and enchant it with Mending to repair the helmet through experience instead of expensive ingots.

Mine Ancient Debris below Y level 16 in the Nether and smelt it in a furnace to get netherite scrap.

Combine 4 netherite scrap with 4 gold ingots in the crafting grid to make one netherite ingot.

Prepare a Diamond Helmet
Craft a diamond helmet from 5 diamonds, or enchant it first with Protection and Respiration so those enchantments carry over.

Upgrade in a smithing table
Place a Netherite Upgrade Smithing Template, the diamond helmet, and one netherite ingot in the smithing table to forge the Netherite Helmet.
Since Minecraft 1.20 the upgrade also requires a Netherite Upgrade Smithing Template; in versions 1.16 to 1.19 only a diamond helmet and a netherite ingot were needed.
