Find an Ancient City in the Deep Dark biome below y=0 and loot its chests to collect Disc Fragment (5) pieces.

Gather nine Disc Fragment (5) pieces; each chest holds only a few, so you may need to raid several Ancient Cities.

Craft the disc
Place all 9 Disc Fragment (5) anywhere in a 3×3 crafting grid to assemble one Music Disc 5.

Play it in a jukebox
Right-click a jukebox with the disc to play the track "5"; right-click again to eject the disc.
Music Disc 5 was added in Java Edition 1.19 and Bedrock Edition 1.19.0. It is the only music disc obtained by crafting fragments rather than as whole loot.
