Ominous Vault is the upgraded Trial Chamber reward block, unlocked with an Ominous Trial Key to drop the rare Heavy Core for the Mace.
Description
Ominous Vault is unlocked with an Ominous Trial Key, dropped after clearing the ominous trial spawners that appear when a player enters a Trial Chamber under Trial Omen (gained by drinking an Ominous Bottle before stepping inside), and each player can claim its loot only once. Ominous Vault is the exclusive source of the Heavy Core (7.5% chance), the ingredient combined with a Breeze Rod to craft the Mace, and also ejects emeralds, diamonds, Wind Charges, enchanted gear, enchanted golden apples, and the Music Disc "Creator". Ominous Vault cannot be obtained in survival - breaking it drops nothing and pistons cannot move it - so a full ominous run should be planned before spending a key.

How to open an Ominous Vault in Minecraft

Get an Ominous Bottle
Obtain an Ominous Bottle from a regular Trial Vault or other Trial Chamber loot; drinking it applies the Bad Omen effect.

Enter with Trial Omen
Drink the Ominous Bottle and step into a Trial Chamber so Bad Omen converts to Trial Omen and turns the trial spawners ominous; the Ominous Vaults are already generated inside, waiting for their keys.

Clear ominous trial spawners
Defeat the stronger mob waves summoned by ominous trial spawners so they drop Ominous Trial Keys.

Unlock the Ominous Vault
Hold an Ominous Trial Key and interact with the Ominous Vault to claim your one-time reward, including a 7.5% chance at the Heavy Core.
Each Ominous Vault can be unlocked only once per player, and a regular Trial Key will not open it — only an Ominous Trial Key works. A Trial Chamber holds roughly 6-8 Ominous Vaults that never convert from normal vaults, so check the deeper combat chambers and crossings if you cannot find one.
